Equine Massage Therapy Waiver and Release of Liability
I, the undersigned, understand and agree to the following terms as a client receiving equine massage therapy services for my horse:
Purpose of Equine Massage Therapy
I understand that equine massage therapy is a non-invasive, complementary therapy designed to promote relaxation, improve circulation, and enhance my horse's overall well-being. It is not intended as a substitute for veterinary care, diagnosis, or treatment.Assumption of Risk
I acknowledge that working with horses involves inherent risks, including injury, illness, or unforeseen reactions to massage therapy. I voluntarily assume all risks associated with equine massage therapy and understand that the therapist is not liable for any injuries or adverse reactions that may occur during or after the session.Release of Liability
I hereby release, waive, and discharge the equine massage therapist, their business, and any affiliated parties from all claims, liabilities, or causes of action arising from the provision of equine massage therapy services. This release includes, but is not limited to, any injury or damage to my horse, myself, or my property.Veterinary Consent
I certify that my horse has been evaluated by a licensed veterinarian and is in suitable condition to receive equine massage therapy. I agree to inform the therapist of any changes in my horse’s health or behavior that may affect the therapy session. I agree that my horse does not have a fever, open wounds, acute trauma, neurological disease, acute rheumatism or arthritis, phlebitis, cancer, fungal skin infections, fistulas, infections diseases or acute stages of viral diseases.Consent to Treatment
I give my consent for the equine massage therapist to perform massage therapy on my horse. I understand that sessions may involve physical manipulation of muscle tissue and stretching, and that my horse may exhibit varying responses to treatment.Photographs and Media Release (Optional)
I consent / do not consent (circle one) to the use of photographs or video of my horse taken during therapy sessions for marketing, educational, or promotional purposes by the equine massage therapist.Cancellation Policy
I understand that cancellations must be made at least 24 hours before the scheduled session. Failure to cancel within this time frame may result in a cancellation fee of $75
